Good Day, I'm an active army soldier relocating to Eglin AFB on 29 July 2011. I'm currently clearing my current duty station (Ft. Bragg), to my understanding, I'm suppose to transport my dependents and mine own medical records to Eglin AFB due to the fact that we will be using the AF hospital located there. I have my medical records but when my wife tried to obtain our children's and her own the Womack Hospital will not release them to her for we are PCS'ing stateside. The record lady stated that I must have the clinic there at Eglin AFB fill out a form to request Womack to mail them to Eglin, is this true? Thank you for your time.

Answer:

Yes sir, the information you received is accurate. Once you and your family PCS to Eglin Air Force Base, you (and/or your spouse) will need take a copy of your orders to the 96th Medical Group's Records Section. The Records technician will complete a form requesting your families' medical records be transferred from Womack to Eglin. This process is in accordance with the DoD policy.
